Children that are not currently in our care are required to have an updated immunization record, lead screening, physical, and updated child information on file with us PRIOR to enrollment...please use the enrollment form found under admissions, or by clicking here.

Enrollment Schedule *
Please select all of the days/times you wish for your child to be in our care. You may change your enrollment schedule, in writing, by emailing Melissa. Please note that our half day program is from 8:00am to 11:30am or 12:30pm to 4:00pm and our full day program is from 7:30am to 4:30pm.

I understand that this 'digital signature' is in lieu of my paper signature. I understand that program availability will be determined by Hawthorn Hill Preschool and my schedule might need to change. I also understand that by completing this form, I am responsible for tuition for my child's schedule according to Hawthorn Hill's 2018-2019 tuition policy. I understand that I must register my child on both Summer and Registration forms if I intend for them to be enrolled throughout these sessions. I also understand that in order for Hawthorn Hill to reserve your child's space, the following must be current: child's file (information, permission, immunizations, lead screening, physical, tuition balance, and registration fees paid). Registration will be on a first come, first serve basis.
